Overview
Chestnut Health Systems is seeking a full-time Recovery Case
Manager for our Peoria, IL facility.

Responsibilities
Provide outreach to referred clients/students to engage them in
behavioral health services, with oversight and guidance as
required.
Provide screening, referral, linkage, and discharge planning
services to recommended services and community resources, and
assist clients in overcoming barriers to needed services, with
oversight and guidance as required.
Obtain appropriate permissions and/or consents for
youth/students.
Provide advocacy, both client and system centered, to ensure that
basic rights of the clients are protected.
Provide services to clients in collaboration with their families
and support systems to support clients with identified behavioral
health disorders in their recovery efforts by coordinating
services, resources, and people to assist the clients, monitoring
the progress of the clients in achieving their established
treatment plan goals. Maintain regular contact with each client's
collaterals (such as school, probation, family, psychiatrist).
Develop working relationships with school officials, probation
officers, and other agents of community resources, and promote
Chestnut services to referral sources.
Attend and participate in all program and team/staff meetings as
assigned.
Document services to clients in accordance with established agency
policies and procedures and in compliance with applicable grant
requirements; required paperwork to be completed and submitted in a
timely manner. Required documentation is to be submitted on the day
services are provided unless otherwise approved by supervisor.
Attend all mandatory Chestnut and interagency trainings and CEU
requirements, and complete all required Relias trainings in a
timely manner.
Promote the recovery model of treatment including trauma-informed
care, hope, respect, empowerment, health/wellness and
spirituality/connectedness by assisting consumers with regaining
control over their own lives through self-directed recovery,
including utilizing recovery language in all applicable
documentation.
Promote Chestnut's culture of customer service excellence through
the adherence to Chestnut's behavioral standards for customer
service.
Qualifications
Must have a high school diploma or equivalent and five (5) years'
supervised behavioral health experience; or a Bachelor's degree in
counseling, social work, psychology, or related human services
field; or a bachelor's degree in any other field with two (2)
years' supervised clinical experience in a behavioral health
setting.
Must obtain certification as Assessment and Referral Specialist
(CARS), Recovery Support Specialist (CRSS), or Alcohol and Drug
Abuse Counselor (CADC) within two (2) years' of being hired.
Requires basic computer skills and experience to include MS Word,
MS Excel, Outlook, and Adobe Acrobat.
Prefer experience with an electronic medical record (EMR).
Must have a valid driver's license with a good driving record,
private automobile insurance and must meet qualifications for
agency auto insurance coverage.
Satisfactory completion of Chestnut's background checking
process.
