SummaryThe Sr. Care Transformation Manager leads the implementation
and monitoring of the MHN clinical integration model of care for
their assigned primary care practices or network. The Sr. Care
Transformation Manager will spend time with their assigned new
geography markets primary care practices care management staff,
their team leads and other frontline staff in 1:1 and small group
interactions teaching teams how to improve and enhance the care
delivery of care management and clinical and practice
transformation services. Emphasis is placed on monitoring quality
of care and staff development in addition to regulatory oversight.
This role plays a critical role in MHN's exciting growth phase and
expansion to new markets. The Sr. Care Transformation Manager
reports to the Vice President, Client Services & Growth and works
closely with MHNs Clinical Integration leadership to leverage the
MHN standard model of care methodologies.Essential FunctionsServes
as a leader member of the Client Services and Growth Team
contributing to the goals and activities of the divisionServes as
the lead or co-lead of client services projects tied to strategic
goals; this spans initiation to completion and includes
establishing the vision, project plan, implementation plan,
communication plan and creation of evaluation activities related to
the projectContributes to the formation and implementation of
clinical and operational initiatives using evidence-based practices
to drive results across the MHN growth service line significantly
improving care and reducing costsAbility to indirectly or directly
supervise staffContributes to the development of the full range of
care management tools, health and wellness tools, and techniques
including health risk assessments, artificial intelligence,
screening tools, and patient and family educational and engagement
tools; Assists sites with their implementationDevelops and provides
education and training of medical home staff, physician and
providers; utilizes webinars and collaborative learning models that
may be disease, patient experience or operationally
focusedResearches innovative ideas and specific programs and
presents findings and develops processes for implementation
following approvalWorks directly with primary care practice
sites/practice networks and oversees the implementation of the MHN
model of care using MHN tools and resources. Evaluate practices
against, established care management/clinical guidelines, practice
transformation guidelines and provide education and consultation
for improvementsOnboard new practice sites/practice networks which
include, but are not limited to, an assessment of the medical
home's/networks capacity to provide care management and a review
and implementation of new primary care practices/networks program
requirements with the organization leadership, provider champion
and care team staffAssists with the design of workflows to support
clinical integration and to maximize the role of care management
staff (care managers and care coordinators)Perform periodic full
program reviews of practices care management processes. Measures
processes against established MHN standards of careMeasure the
quality of care management delivered at the practice level through
live staff interactions and patient reviews and make
recommendations for improvement. Oversee that reccomendtions are
implemented as appropiategIntegrate the full range of MHN
technology solutions and data analytics, work with sit site-based
care management staff to utilize information produced by MHN and
other available data to enhance performance and qualityContribute
to the formation and implementation of clinical and operational
initiatives using evidence-based practices to drive results across
the MHN population significantly improving care and reducing
costsContribute to MHN playbook which includes the development of
the full range of disease management tools, health and wellness
tools, and techniques including health risk assessments, screening
tools, point of care reminders, and patient and family educational
and engagement tools; assists sites with their
implementation.Utilize MHNs standard model of care tools to develop
and provide education and training to primary care
practices/networks staff and provider champions; utilize
collaborative and engaging educational models that effect retention
and implementation of learnings into practiceManagement of AI high
risk stratificationprocess. Training and development relative to
designing efficient processes to managing:Care Management (CM)
caseloadsOnboarding and discharging patients from care
managementOptimizating how the care management team works
(collaboration between CC/CM, PCPs and other
clinical/administrative supports)Ensuring care manager delegation
process (CM step-down process)Provide primary care practices with
staffing review and support through comprehensive live case
coaching, ensuring proactive review of current cases based on
identified standards of care and tools, and integrating meaningful
chart review systemsRoutine care management performance oversight
using established standardized reportsTraining program participant:
Plan and deliver trainings under direction of training managerHold
standing meetings with designated members of primary care
practices/ network executive leadershipCompetenciesStrong
knowledge, skills and demonstrated completion of coursework in
quality improvement, and project and data management.Demonstrated
knowledge and experience with electronic information
systemsDemonstrated knowledge and experience of systems to improve
population health and disease managementDemonstrated knowledge and
experience in management of disease processesExcellent oral,
written and interpersonal communication skills. Ability to work
independently and also as a team with a wide variety of
individuals. Excellent organizational skillsKnowledge and
experience working with NCQA, Medicaid, Medicare standards and
requirements.Demonstrated experience in program development,
training/education, managementAbilities to foster teamwork among
physicians and staff at all levels throughout an
organizationExperience in academic medical centers, safety
net/public health hospitals, FQHCs and physician groups
desirableProficient computer skillsRequired EducationBachelors
degree required in Nursing or Masters degree in Social Work with
current licensure.Additional Eligibility RequirementsMinimum of 5-8
years of healthcare experience focused on Clinical
Integration/Practice Transformation, Hospitals, Ambulatory Care,
Physician group; a combination preferred.At least 3-5 years
leadership, consulting or customer/patient relationship, health
