Job Description:
Shifts: 8 hour days
Department/Unit - Rehabalitation
Performs examination and evaluation of patients' physical and
functional status including musculoskeletal, neurological,
cardiopulmonary and integumentary systems as they relate to
functional mobility skills.
Includes gathering information from the patient chart, patient
and/or family, or other source as appropriate.
Analyze evaluations results, establishes PT diagnosis/prognosis,
sets appropriate short and long term goals, and designs an
individualized plan of care.
Selects, administers, and monitors treatment interventions as
appropriate.
Reassesses patients' responses and/or outcomes to intervention,
adjusts program accordingly, and coordinates treatment with
patients, families, physical therapist assistants, and other health
care disciplines.
Communicates treatment changes to physician as appropriate.
Recommends otehr disciplines, programs, agencies and equipment to
patient and/or family, as appropriate.
Plans and documents appropriate home programs.
Instructs patients and patient's families in appropriate continued
exercise or therapy programs and in use of therapy techniques and
assistive devices.
Collaborates with interdisciplinary team to improve quality of care
and facilitate discharge planning.
Maintains personal productivity at or above department standard.
Supervises, orients and evaluates students, orients and trains new
department staff and volunteers as assigned. Maintains accurate and
timely documentation including evaluations, re-evaluations,
progress notes, discharge summaries, patient education, and
prepares documents to communicate to appropriate care team Mission
Partners.
Attends patient review conferences, clinicals, in-service education
classes and meeting as necessary.
Attends mandatory department, Medical Center meetings, and other
work-related educational programs.
Maintains knowledge of current trends and developments in the field
and incorporates evidence-based practice into the delivery of
care.
Bed Size: 81 bed acute care
State license status must be currently active
Minimum of 1 year experience required
Certifications:
BLS, BSN
Skills:
Ultrasound, Estim units, treadmills, recumbent steppers, UBE's,
Epic EMR, iontophoresis
