Job Description:
Oncology Dietitians are primarily responsible for providing
comprehensive nutrition care, Medical Nutrition Therapy (MNT), and
nutrition education for oncology patients at multiple clinic sites
throughout the region. This position develops and implements
best-practice strategies to improve the health of individuals and
communities, primarily focusing on oncology-specific dietetics.
Responsibilities include planning, administering, and managing
programs that improve the health of individuals impacted by cancer
and the community, both one-on-one and community-based group
nutrition education for adults; maintaining grant and contract
requirements, program budgets, and expenses; integration of
nutritional programming and services throughout the cancer care
continuum. Dietitians serve as a health resource for the community
and support the community in behavior change through promoting and
advocating for public health.
- Provides comprehensive nutrition care and Medical Nutrition
Therapy for oncology patients according to policies & procedures.
Actively collaborate with multidisciplinary team to deliver
outstanding patient care. Provides nutrition education through
community-based classes and public health interventions in
individual and group settings both on-site throughout the
community.* Responsible for Medical Nutrition Therapy sessions for
oncology patients per policy* Administers medical nutrition therapy
by nutrition assessment, modified diet formulation, self-management
training, and nutrition intervention to oncology patients*
Responsible for outpatient/community nutritional assessment
completion per policy* Applies the Nutrition Care Process using
ADIME (nutrition assessment, identification of nutritional
diagnosis, implementation of appropriate nutrition intervention,
and formulation of patient specific goals using
monitoring/evaluation) when appropriate.* Meets charting guidelines
100% of the time determined by chart audits* Provides
evidenced-based nutrition support as appropriate for patient care*
Ability to analyze performance compared to regulations and offers
ways to better, efficiently comply to provide optimum nutrition
care* Fulfills CDR and IDPR education requirements and maintains
report portfolio with education receipts for IDPH or CDR audit*
Provide support for patients across the cancer continuum * Act as a
liaison among the patient, family, caregivers, and healthcare team*
Provide comprehensive documentation of patient encounters,
education and referrals* Provide and reinforce education in all
phases of cancer continuum including, but not limited to:
treatment, care plan, symptom management and survivorship concerns*
Develop oncology-related education materials* Contribute to the
knowledge base of the healthcare community through involvement in
professional organizations, presentations, publications and
research* Attend community health fairs and screenings; provides
community education presentations as appropriate* Collaborate &
communicate frequently and consistently with providers & other
appropriate healthcare team members, ensuring seamless plan of care
and follow-up care* Provide telephone triage services (e.g. symptom
management, education, resource referral) for patients/families*
Facilitate communication with patients, survivors, families and the
health care providers to monitor patient satisfaction with the
cancer care experience and contributes to the development of action
plans and implementation of plans* Ensure patient adherence with
treatment plan* Maintain an active role in disease specific Tumor
Conferences, including follow up on recommendations* Utilize
appropriate screening and assessment tools (e.g., distress
screening, etc.)* Maintain/use a comprehensive database of local,
regional and national resources* Understand resources available to
patients experiencing financial hardships and/or are
uninsured/underinsured * Facilitate individualized care based on
culture, health literacy, ethics, psychosocial needs, etc.* Ensures
adequate communication during hand-offs or transfer of care* All
other duties as assigned* Plans, implements, and evaluates public
health nutrition education programs to impact attitudes, knowledge,
and behaviors for improved health using evidence-based and
research-based curriculum as appropriate* Applies evidence-informed
theories, models, and/or strategies to health promotion programs. *
Identifies, develops, delivers, and evaluates health communication
messages using a variety of communication strategies, methods, and
techniques* Provides expert consultation, assistance, and guidance
to individuals, groups, and organizations.* Develops tools for
nutrition education and promotion of healthy eating for patients
with a cancer diagnosis* Creates an environment conducive to
learning for a variety of diverse populations and in diverse
settings* Represents the interests of
