Overview Implements plan of care as provided by the physical
therapist, treating patients of all ages. Serves as a role model in
delivery of professional services and as a clinical resource for
staff and students. This position qualifies for a sign-on bonus!
Qualifications Education Level Field of Study Associate's Degree
Physical Therapy Assistant Licenses/Certifications Licensed
Physical Therapist Assistant (PTA) - Illinois Department of
Financial and Professional Regulation (IDFPR) And Basic Life
Support (BLS) within 30 days - American Heart Association (AHA)
Attend and satisfactorily complete all required continuing
education regarding the care of acute stroke patients - *Required
to successfully complete board exam on the next available exam date
- Continuing education requirements for this position may include
one (1) hour, four (4) hours, or eight (8) hours of content that is
assigned by the organization to comply with DNV Hip and Knee
Replacement Certification requirements. This content will only
apply to those directly assigned to the total joint replacement
practices and will be communicated to the applicable staff member
by the immediate supervisor. This requirement is subject to change
pending updates to DNV Hip and Knee Replacement Certification
standards. Responsibilities Essential Functions Educates patients,
family, caregivers, students, and other health care providers using
relevant and effective teaching methods. Follows the therapist's
plan of care. Adjusts the treatment based on patient response and
available evidence only after communicating with the evaluating
therapist and documenting that communication in the daily note.
Produces documentation in accordance with the Clinical
Documentation Policy (CTSCP67). (23) If applicable, completes
annual education as assigned to comply with DNV Hip and Knee
Replacement Program requirements. Department Specific Job Function
Implements intervention plants according to the treatment plan
established by the Physical Therapist, utilizing activities that
are functional in nature and take into consideration the functional
and emotional needs of the patient/family. Modified approach in
response to patient behavior and performance. Utilized standardized
outcomes measures in a uniform manner to ensure functional
performance is accurately reflected. Completes documentation in
accordance with regulatory and organizational standards.
Collaborates with patient, family and significant others throughout
the rehabilitation process and provides education/family training
as necessary. Communicates with supervising Occupational Therapist
when there are any changes in patient status, change in functional
performance, or requested change to the treatment plan. Clearly
communicates patient performance, change in status,
activity/transfer recommendations, and recommendations for
discharge disposition and equipment required for the next level of

this position is $22.4per hour - $37.41per hour. This represents a
good faith minimum and maximum range for the role at the time of
posting by Carle Health. The actual compensation offered a
candidate will be dependent on a variety of factors including, but
not limited to, the candidate’s experience, qualifications,
location, training, licenses, shifts worked and compensation model.
