Job Description Job Description Location: OSF Divine Mercy Continuing Care Hospital, Peoria, IL Department: LTAC Med Surge Shift: 12-hour Nights Position Overview The RN will provide high-quality patient care in a Long-Term Acute Care (LTAC) setting using the nursing process of assessment, planning, intervention, and evaluation. This role collaborates with patients, families, physicians, and interdisciplinary staff to ensure effective care delivery. Key Responsibilities Deliver direct and indirect patient care following hospital and nursing standards. Assess, plan, implement, and evaluate patient care plans. Document assessments, care plans, treatments, and progress in the medical record. Supervise and assign tasks to nursing team members based on patient needs and staff qualifications. Administer medications safely and accurately. Assist physicians during exams and procedures. Educate patients and families on treatment plans, pain management, and recovery. Communicate changes in patient condition to providers and leadership. Participate in discharge planning and interdisciplinary coordination. Required Qualifications Active RN license (Illinois preferred). BLS certification required; ACLS preferred. Minimum 1 year RN experience (Med/Surg preferred). New grads considered on a case-by-case basis. Must pass a full drug screen (OSF is a Drug-Free Campus). Education Graduate of an accredited Nursing Diploma, ADN, or BSN program. Skills & Abilities Strong communication and interpersonal skills. Knowledge of safe medication administration. Ability to prioritize, multitask, and work in a fast-paced LTAC environment. Basic computer proficiency. Team-oriented with reliable attendance.
