Job Description:
Overview Provides restorative, rehabilitative, and recreational
services to patients on the following areas: behavior health
(inpatient and partial hospitalization), rehabilitation unit and
skilled nursing unit; consultative and prn services provided to
pediatrics. Qualifications Educational Requirements Bachelor's
Degree in in therapeutic recreation, music therapy, activity
director. Certification & Licensure Requirements - Certified
Alcohol & Drug Abuse Counselor (CADC). - For staff hired into this
position after February 27, 2022, certification (CTRS, MT-BC, or
ADC) must be obtained within 1 year of employment and be maintained
annually. Upon Hire - Crisis Prevention Institute (CPI Training) If
hired after August 1, 2019 - CPI within 3 Months - Use of usual and
customary equipment used to perform essential functions of the
position Upon Hire - Work may occasionally require travel to other
UPH facilities/hospitals Upon Hire - Required to drive your own
vehicle for business purposes. Upon Hire Other Requirements Writes,
reads, comprehends and speaks fluent English. Basic computer
knowledge using word processing, spreadsheet, email, and web
browser. Multicultural sensitivity Critical thinking skills using
independent judgment in making decisions. Advanced reading, writing
and oral skills Ability to utilize PC including, basic elements of
Windows environment, keyboarding, work with mouse, and perform
basic computer tasks with instruction. Responsibilities Conducts
initial and ongoing assessments with patients and documentation is
timely, complete, legible, concise, and related to the identified
needs of the patients. - Conducts thorough initial assessments
within time frame indicated in department- specific policy -
Documentation is legible, concise, and clinically appropriate -
Demonstrates awareness of appropriate assessment tools/techniques
and their application - Conducts ongoing assessments as appropriate
- Input on care path is timely, legible, concise, and clinically
appropriate. - Progress notes relate to patient needs/problems
identified in the care path - Progress notes reflect staff
intervention as well as patient response (effectiveness of
intervention) utilizing DAIR format. - Progress notes are timely
and meet the frequency as stipulated in the department specific
policies. Provides therapeutic services for patients. - Individual
or group activities occur on time and as scheduled - Services are
individualized to the needs/functional abilities of the current
patients - Services provided are targeted to the needs identified
in the patients' care paths - Services provided are evidence based
reflecting current and accepted techniques and are of high quality
- Seeks clinical supervision when appropriate - Makes appropriate
referrals for ancillary/supportive services. - Takes all necessary
precautions and risk management steps to protect patients' safety
at all times. - Group facilitation skills reflect understanding of
group dynamics as well as dynamics of individual
diseases/disorders. - Assures that therapeutic activity therapy
supplies are available to meet patient needs. - Demonstrates
knowledge of developmental levels as well as age-appropriate
activities. - Designs and implements community re-integration
activities. - Uses CPI methods when dealing with agitated,
aggressive patients Actively participates actively in the
multidisciplinary team approach to care. - Participates in team
staff meetings - participates in multidisciplinary staff meeting -
Documents plan of care-on-care path - Communicates timely and
appropriately with other members of the treatment team regarding
the patients' progress - Advocates for needs of patients - Uses
informal opportunities to collaborate with other disciplines - Uses
creativity and knowledge of disease and recovery, in planning care
for patients - Serves on interdisciplinary committees as requested
by Nurse Manager. Participates in quality improvement activities -
Identifies areas for improvement related to Therapeutic Activity
Services (TAS) role in the care of patients - Participates in data
collection for quality improvement purposes - Participates in
development and implementation of action plans for quality
improvement - Demonstrates initiative in the improvement of TAS
services - Demonstrates initiative in his/her professional growth

The compensation range for this position is $21.25per hour -
$35.49per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate’s
experience, qualifications, location, training, licenses, shifts
