Job Description:
Job DescriptionOur Group Respite Program is looking for a fun,
energetic person to plan small group activities for people that
reside in the community! Schedule can be very flexible based on
activities/events planned such as swimming, movies, exercise and
community exploration! PURPOSE: The Community Direct Support
Professional is responsible for coordination, supervision,
documentation, and provision of personal support to participants in
community-based support programs. This includes demonstrating
initiative to create, manage, and maintain a flexible schedule that
reflects needs and requested support hours of the persons receiving
services on assigned caseload. ESSENTIAL JOB FUNCTIONS:Must present
suitable role modeling for person served and guide their own
behavior in a professional, empathetic, and caring
manner.Demonstrate initiative to problem solve and employ sound
independent judgement while providing community-based services.
Requires high level of organization and reliability to ensure
documentation and communication is accurate and completed in a
timely fashion. Requires regular communication with your supervisor
despite being a field-based worker. Responsible to create, manage
and maintain a work schedule based on needs and requests of persons
served. Provide protective oversight balanced with the dignity of
risk to support opportunities for growth and development. Report
abuse and/or neglect of persons receiving services per policy and
procedures. Perform safety and emergency procedures accurately and
calmly, including First Aid/CPR. Consistently practice infection
control measures.All interactions with others should demonstrate a
supportive, therapeutic and cooperative attitude. DUTIES AND
RESPONSIBILITIES:Assist person receiving services in providing
input into the development of the supports and services plan and
schedule. Willing to work a flexible schedule to meet individuals
needs that may include nights, weekends and holidays.Be responsible
for implementation of plan outcomes and documentation. Must be able
to complete data entry using a tablet, laptop, or basic desktop
computer.Support persons receiving services in and across
environments, home and community, in order to assist them in
maintaining maximum independence. Assistance to identify and
participate in various community-based activities individually or
in small groups. Cross training on other caseload support needs,
schedules and activities is required.Maintain and be responsible
for accuracy and promptness of all requested documentation,
including billing notes, support documentation, travel logs,
accidents, illness, incidents, health monitoring, etc. Complete
training as assigned by supervisor. Demonstrate knowledge and
adherence to agency policies, procedures, persons served rights,
concepts of normalization and respect for human dignity. Be an
advocate and empower self-determination, skill development and
community connections.Qualifications:High School Diploma or GED (no
online High School Diplomas will be accepted).Valid driver's
license. Pass a physical, drug screen, and fingerprint background
