Digital Acute Care Registered Nurse is a member of a collaborative
team. This position is responsible for providing virtual and
in-person nursing care to a diverse population of adult
patients/clients. Care is provided through a combination of
telehealth technology and hands-on care in the patient's home
environment. The nurse provides proactive monitoring, prevention,
early intervention, and ongoing care. The goal is to optimize the
quality of life for the patient at home. Access to professional
acute, inpatient healthcare services is a priority. Evidence-based
care and clinical decision support are utilized to guide practice.
Ongoing communication is performed to promote quality of care.
Collaboration occurs with all service team members, patients, and
